Introduction: Recurrent acute pancreatitis (RAP) is a clinical condition characterized by repeated acute pancreatitis (AP). The most frequent causes are gallstones and alcoholism. However, when the cause cannot be determined, which occurs in 10% to 25% of patients, it is known as idiopathic recurrent acute pancreatitis (IRAP). Caso report: We present a 32-year-old female patient with two episodes of AP, with a history of laparoscopic cholecystectomy. The patient presented a new episode of severe RAP that required in-hospital treatment. Microlithiasis was identified as the cause, and an endoscopic retrograde cholangiopancreatography (ERCP) and sphincterotomy were performed a few months after the acute condition. Conclusion: Microlithiasis, hyperkalemia, hypertriglyceridemia, and drug-induction are uncommon causes of idiopathic recurrent acute pancreatitis.
