Introduction: Osteochondritis dissecans of the knee (ODK) is a condition in which a subchondral bone fragment and its articular cartilage are separated from the overlying bone. It most often affects teenagers and young adults. If the free fragment cannot be repaired, excision should be performed, having the following options: microfractures or perforations of the viable and bleeding subchondral bone, mosaicplasty, osteochondral allograft, and regeneration with autologous chondrocytes. Description of the case: We present the case of a 9-year-old male patient with a diagnosis of osteochondritis dissecans of the knee 2B of Cahill and Beng/Harding, who underwent arthroscopic mosaicplasty in June 2017. After a 27-month evolution, he has been integrated into his daily life activities with complete mobility arches, without pain, and with a score on the Lysholm Knee scale of 95 points. Conclusion: In this case, mosaicplasty through arthroscopy proved to be a safe and effective treatment, with good functional results in the short and long term; therefore, it could be considered an option in the treatment of skeletally immature patients with ODK. Further prospective studies of his technique in similar patients are needed to confirm or rule out this therapeutic option.
