Open access medical journals are a strategic component for the democratization of scientific knowledge and the strengthening of national research. This editorial analyzes the importance of consolidating institutional models of editorial sustainability, based on the recent Guidelines of the National System of Scientific and Humanistic Publications in Mexico. It highlights the need to guarantee continuity, editorial quality, and non-commercial open access through the commitment of publishing institutions. It also recognizes the role of initiatives such as Latindex, Redalyc, and SciELO in preserving bibliodiversity, as well as the relevance of the Diamond open access model for a more equitable and inclusive science.
