The Physiotherapeutic approach in Legg-Calvé-Perthes

Introduction: Legg-Calvé-Perthes disease is a pediatric condition characterized by necrosis of the femoral head due to an interruption of blood supply, present in children aged 2 to 8 years and with a greater predisposition to boys. Despite the amount of literature, its etiology remains unknown. Objective: To provide an updated review of the disease and analyze the evidence on the optimal physiotherapeutic approach to contribute to a better prognosis and quality of life. Methods: A narrative review with structured search was conducted in databases (PubMed, SciELO, Google Scholar) between 2010 and 2024. Thirty-three articles focusing on conservative interventions were included and analyzed qualitatively. Results: The physiotherapeutic approach demonstrated significant benefits in improving joint range, muscle strength, and pain reduction. However, great heterogeneity and methodological limitations were identified in current studies, with expert consensuses predominating over controlled clinical trials. Conclusion: Physiotherapy is a fundamental pillar in the treatment that contributes to a better functional prognosis. However, it is imperative to conduct studies with greater scientific rigor to standardize exercise doses and protocols according to the stage of the disease.
